学生体检指南Instruction on Physical Examination

1、地点：成都市桐梓林北路1号-四川国际旅行卫生保健中心

Address：Sichuan International Travel Healthcare Center, No.1 Tong Zi Lin Road North, Chengdu, P.R.China

4号线（成都大学站）—换乘1号线（骡马市）—桐梓林（B口出）

Metro No.4(Chengdu University Station)—Metro No.1(change at Luomashi Station)—Tongzilin Station(Exit B)

2、注意事项Notes：

（1）空腹，不吃早饭、不喝水 Do not eat and drink anything on the day.

（2）建议早上8点或更早出发，周末不上班，平时要在十点前到。 Suggesting to go on 8 am or before. Only on weekdays, better arrive before 10 am.

（3）2寸证件照3张. Three 2-inch ID photo are needed.

（4）携带护照. Bring with passport.

（5）费用：508元左右，建议带600元。

 Fee: around 508 RMB, 600RMB is suggested.

（6）检查缴费结束后，医院会给出发票和一张写有领取体检结果时间的单子，请按照单子上的时间领取体检结果，或者付邮费请医院邮寄。

After the payment, the hospital will give you an invoice and a receipt with the time to pick up your result.Please go to collect the result on time or pay the delivery to ask the hospital to send you the result.
